I accept that my personal data in general and special categories will be processed and transferred by DENT GLOW AĞIZ VE DİŞ SAĞLIĞI POLİKLİNİĞİ (Dent Glow Clinic and briefly “Clinic”) within the scope of the Turkish Personal Data Protection Law No.6698 (“PDPL”), European Union General Data Protection Regulation (“GDPR”) and the clarification text provided to me, curing the following cases:

I approve processing and transferring my personal data to domestic service providers who are in the position of data processor, for the purposes of creating and monitoring my patient record in electronic environment, recording my diagnosis and treatment, operation, and medical procedure data, providing electronic data storage, back up services or systems.

I approve processing of my personal data in order to measure my satisfaction regarding the services by questionnaires regarding the clinical services and to take the necessary actions and perform analyses for my satisfaction. 

I approve processing of my personal data for the purposes of sharing on the Clinical website within the scope of conducting advertising, marketing and promotional activities related to the products and/ or services and transferring my data to the third parties and domestic service providers through the website.

I approve processing of my personal data in order to share them on publicly opened social media accounts within the scope of conducting advertising, marketing, and promotional activities, and transferring them to the third parties and Foreign Service providers via social media accounts (Instagram, Facebook, Twitter, YouTube etc.)

I approve the transfer of my personal data to service providers whose servers are located abroad for the purposes of carrying out storage and archiving processes, providing cloud, corporate e-mail services, electronic or physical data storage, back up services or systems.

I approve the transfer of my personal data to foreign-based communication channels (WhatsApp) in order to carry out communication activities.

I approve the transfer of my personal data in general and special categories to service providers whose servers are located abroad, when it is necessary, for the purposes of carrying out communication activities, carrying out storage and archiving processes, providing corporate e-mail services, providing electronic or physical data storage, backup services or systems.

Within the scope of the cases stated in this Explicit Consent text, your personal data will be processed based on the legal reason of “explicit consent” regulated in the provisions of articles 5/1 and 6/2 of PDPL and articles 6/1 (a) of the GDPR. By applying to our company, you always have the right to withdraw your explicit consent for the processing of your personal data.
